Bring your insurance card, photo ID, and physical/occupational therapy prescription from the doctor ordering your care (if required). If you are a post-surgery patient, bring any information you were given by your surgeon or PA, such as treatment protocols, exercises you were given the day of your procedure, and an updated list of all medication you are currently taking.
